
Community Mental Health Care Support Worker

The post holder will work under the overall supervision of a qualified member of staff and will undertake work which consists of direct client contact as directed. For much of the time, the post holder will be expected to work alone, without the direct supervision from a qualified nurse.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
To perform tasks within the framework of a Care and Treatment Plan as set and evaluated by a qualified practitioner at appropriate intervals.
When working without direct supervision, the post holder will be required to make judgements in clinical situations as to the sharing of clinical information with other team members.
As a community based Healthcare Assistant, the post holder will be required to carry a caseload of clients under the indirect supervision of qualified team members. In doing so, they will be required to prioritise their workload effectively.
To maintain the confidentiality and appropriate information at all times.
The post holder may be required to organise therapeutic groups for client activities, either alone or in partnership with other team members.
To keep timely and accurate clinical records for statistical and clinical purposes.
To reinforce health promotion advice including lifestyle choices as directed by the qualified nurse and other team members in accordance with NICE guidelines.
On occasions the post may be required to convey medication from the pharmacy to the CMHT, or from the CMHT to the patient’s home under the direction of the qualified nurse.

The post holder will be required to understand a range of work procedures gained through training and experience befitting of the band in order to be able to work without direct supervision from a qualified nurse.
When working without direct supervision, the post holder will be required to make judgements in clinical situations as to the sharing of clinical information with other team members in particular, the qualified nurse supervisor, which will either need further intervention or require further discussion and recording in the CMHT.
As a community based Healthcare Assistant, the post holder will be required to carry a caseload of clients under the indirect supervision of qualified team members. In doing so, they will be required to prioritise their workload effectively on a day to day basis including the planning and organisation of tasks and activities and programs as indicated in the clients care and treatment plan.
