We are looking to recruit a highly motivated, innovative and enthusiasticPsychosocial Workerto join ourIntegrated Healthcare Serviceteam at HMYOI Cookham Wood. At HMYOI Cookham Wood we care for 15-18-year-old boys in custody; we are looking for staff that understand their needs and work in line with ‘Child First’ principles. Working in partnership with the criminal justice system and other agencies, we provide responsive, dependable and high-quality primary care and mental health services to existing offenders in the communities we serve, to improve their health status and to encourage social inclusion.

The Psychosocial Worker will use their specialist knowledge and skills of substance misuse to undertake assessment, provide psychosocial support and care-planning of individuals under the care of the IHWBT, whilst implementing and evaluating treatment of clients. To hold a caseload of clients under the supervision of the Senior Psychosocial Worker. To contribute to the SECURE STAIRS framework by participating in and delivering training, consulting to and supporting custodial staff, and participating in Formulations, Reflective Practice and other key activities that contribute to the development of therapeutic cultures on the units at HMYOI Cookham Wood.

**MAIN DUTIES AND RESPONSIBILITIES**:
**Clinical Responsibilities**:
1. To work as a member of the Wellbeing team, which provides a range of specialist services to substance users and those with psychological and mental health needs, including harm minimisation approaches, motivational interviewing, relapse prevention, care planning, health education and detoxification support, mindfulness and groupwork.
2. To make continuous assessments of the psychological, emotional, social, and rehabilitative needs of young people and young adults, considering the needs of family, local community and sentence plan, with regular evaluation of care plans.
3. To provide case management to a defined caseload of clients, under the supervision of the Senior Psychosocial Worker, working in a trauma informed approach.
4. To provide Psychosocial health promotional education to Young People with a secure education setting.
5. Contribute to the integrated landing-based health support team for an identified landing, including being a visible presence on the landing and developing good working relationship with young people and staff.
6. To use agreed and established outcome monitoring tools and any other relevant quality measures.
7. To manage the transfer of individuals between prisons and into the community.
8. To counsel individuals about their substance use and/or harmful behaviours, using motivational interviewing and other evidence-based interventions.
9. To ensure full risk assessment and risk management is delivered effectively.
10. To understand and implement clinical governance.
11. To ensure the highest quality of service is delivered by following local and national quality and performance standards, guidance, procedures, and practice guidelines.
12. To ensure that all work is performed in accordance with Trust policies and procedures, including Incident Reporting.
