Housing Support Worker

**About us**
**Pivotal**:
- Provides sustainable homes, specialist support and compassionate care to vulnerable people.
- Works closely with local authorities and other relevant commissioners, such as the NHS.
- Provides approximately 450 homes for vulnerable people.
- Offers vulnerable people the opportunity to rebuild their lives or have a better quality and independent life.
- Operates successfully in a number of locations, including Cornwall, and Dorset.
**Key Responsibilities**:
- To ensure that the service provides a safe, client focussed environment that utilises a holistic approach to support.
- Provide practical and emotional support to clients; giving guidance on issues as they arise.
- Provide a holistic approach in supporting clients to achieve the goals outlined in their support plan.
- Effectively engage and empower clients to review choices and behaviours and make positive changes to their lifestyle; working in partnership with other services as appropriate.
- To support the client to maintain their tenancy and to develop the skills required to live independently in the future.
- Work with the housing management team to assist the client with move on at the point that they are ready for service exit.
- To assess needs and risk in partnership with the client; ensuring that support and risk management plans are robust.
- To regularly review support and risk management plans ensuring that the support aims are in line with the clients needs and wishes, and that the level of support provided is suitable.
- Work in partnership with the housing management team to ensure best possible outcomes for customers.
- Ensure that all required documentation is accurately maintained and up to date.
- Ensure that safeguarding’s, incidents, accidents and near misses are reported and responded to appropriately.
- Support the Project Manager in ensuring that the service meets it’s key performance indicators, addressing any barriers appropriately.
- Provide data as required by the Support Project Manager.
- Report all maintenance and health and safety concerns to the Housing Management team and support clients to do so.
- Support and encourage clients to attend house meetings..
- Assist your manager in responding to internal / external audit ensuring that any actions are completed in a timely manner.
- DBS checks are required for roles where you will be working with or have lone contact with Vulnerable Adults
**We also have a role available that involves full mobility across our properties in Dorset. Milage will be paid at 45p per mile.**
**EDUCATION, TRAINING, SKILLS AND WORK EXPERIENCE**
- Understanding of safeguarding practice
- Passionate abut working with vulnerable people
- Good level of literacy, numeracy and I.T skills.
- Good verbal and written communication skills.

- Able to travel independently and effectively throughout the company’s area of operation
