We are looking for a motivated and caring individual, preferably with some community experience, but more importantly someone who is enthusiastic about rehabilitation with older people, providing support to people who have long terms needs and who enjoys working in a team.
The Community Rehabilitation Team is a friendly multidisciplinary team, with staff who work together to facilitate early discharge from hospital or prevent unnecessary admission.
A short animation has been developed to explain why we need ICCs and how they will work: youtu.be/DeysdmvzHoEHi
A rehabilitation assistant in a multi-disciplinary team is expected to work with other health and social care professionals to develop the ethos of multi-disciplinary, integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.
As a rehabilitation assistant reporting to a registered health professional, the post holder will implement rehabilitation plans for patients in the community and other settings, observe the patient’s condition, providing direct care with a rehabilitation and therapy focus. This may include supporting patients with personal hygiene and activities of daily living.
Flexible working will be required to meet the needs of the patient group.
The ability to work over 7 days is a requirement of the post. The hours of work will be on a rota covering a shift pattern between the hours of 8.00 a.m. and 6 p.m.
Continuing professional development is actively encouraged and as part of the team you will have the opportunity to take part in the process.
Car driver essential.
