**Female* Community Link Worker**
An exciting opportunity has arisen to join our Health and Advocacy Service based in HMP Bronzefield. This service aims to identify and work with people going through the criminal justice system who have mental health needs and other vulnerabilities to support their access to appropriate services whilst in custody and in the community. This role offers the opportunity to work with women on remand with a variety of needs including prenatal and perinatal needs, making a real difference to their lives whilst in custody and beyond.
You will be working closely with Women in Prison (WiP), the prison based healthcare team and Probation to support vulnerable women in a forensic setting who often get overlooked, may feel unheard, and who without services like ours, may not have appropriate care in place before being released into the community. You will be in a position to support and deliver early intervention and services for women and girls in contact with the Criminal Justice System including Diversion and Out of Court Disposals, and make a positive difference in their lives by reducing reoffending, and making physical and mental healthcare services more accessible. You would also be involved in Together for Mental Wellbeing’s women’s champion strategy group where you will be part of initiating change and learning to our current practice.
**Key functions include**:
**Offer short-term liaison and support through**:
- **Completing initial needs assessment to identify any mental health, physical health and substance misuse needs.**:
- **Contributing to the management of risk for the person, including raising safeguarding concerns for risk to self, others or from others.**:
- **Referring women to **specialist female offending and health care services and making sure referrals are followed through.**:
- **Signposting and providing information where relevant.**:
- **Supporting the provision of resettlement services i.e. Accommodation, Finance Benefit and Debt as well as Health Advocacy.**:
- **Coordinating and working in partnership with WiP, NHS, and The Probation Service in prison and in the community.**

- The above is a full time (37.5 hrs a week) post, but we can consider part-time arrangements. The role is primarily based in HMP Bronzefield.
