Social Prescriber

Provide information and support to service users to enable and empower them to access services and engage in activities and behaviours to enhance their wellbeing. Undertake assessment of new referrals. Co-design a support plan with service users. Maintain a close working relationship with the Advanced Health Improvement Practitioner to liaise over client support plans.
Agree appointments and contact arrangements with service users to review progress and identify further support needed. Maintain case notes on System 1. Implement closure of cases when the needs of the service user have been met or if they decline to take up the service. Inform the Senior Health Improvement Practitioner or FUSE Lead of issues concerning the immediate safety or wellbeing of service users.
Attend supervision/team meetings. Monitor and evaluate outcomes through specified processes, procedures, and systems. Give feedback on performance and local intelligence to lead manager and head of service. Establish working relationships with ED staff primary care staff at surgeries and Voluntary and Community Sector organisations providing services that may benefit people in our service caseload.
Adhere to FUSE policies and procedures. Undertake any training identified as needed by the Senior Health Improvement Practitioner or FUSE Lead. Assisting with the provision of information and support to the Advanced Health Improvement Practitioners. Observe Livewell SW policies in respect of equal opportunities, health and safety, confidentiality and data protection.
Develop, promote and co-ordinate health activities and other community engagement events to deliver health promotion and brief interventions
- signposting multi-Agency organisations and voluntary sector. Be up to date with a range of health improvement topics through competency based learning.
