The Forensic Outreach and Liaison Service (FOLS) is a community-based service in Sussex providing holistic support to individuals with a mental health that relates to their offending/risk behaviour.

Our service is seeking a dynamic, enthusiastic, and committed peer support worker to support our clients living in the community and in their transition into the community.

The Peer Support Worker role is a vital part of our plan to continually improve the service that we provide and is aimed at people who have lived experience of using forensic health care services or related experience in other mental health services. We are looking for people who can use their experience of mental distress, recovery and use of peer networks to inspire others.

**We have a strong supervision culture**: your work will be coordinated, supported day-to-day, and supervised by experienced, professionally qualified staff.

You will be employed as Band 3 Peer Support Worker for 37.5 hours per week on a substantive contract.Based inSussexyou will offer more intensive support to be at the point of discharge. This will include direct work with the client around activities of daily living, developing meaningful community activities to engage in and directly reporting into the FOLS team. You will also support the community provider by increasing the presence of specialist mental health services to help manage any difficulties that may arise at the point of transition.

Support workers provide time to an allocated group of clients and thus promote their recovery, helping them to maintain their independence in the community. As a support worker you are part of a team which provides mental health interventions, and your role is to focus on the day
- to-day holistic support needs of service users.

You will be required to adhere to the team and Trust’s lone-working policies at all times.

You will be interested in supporting the service user's transition into the community following an inpatient admission. You will already have some experience of working with and supporting these client groups.

**Your key activities will include**:

- To assist clients by acting as a bridge between services, ensuring smooth transitions of care
- To work with the multi-disciplinary team to establish and deliver a therapeutic programme of recovery for clients with learning disability and autism needs, using CPA principles, evidence based and client centred approaches
- To be responsible for liaising with and providing information to other agencies involved in the support of the clients.
- To develop meaningful relationships with clients within appropriate and transparent boundaries, and to seek support and use supervision to facilitate these relationships.

This role will involve travel and can at times require you to respond dynamically to support your team colleagues in other locations using both remote working methods and attending in person as required.

**Pay Band**: AFC B3 (, and travel expenses are paid according to AFC)

FOLS operational hours 37.5 hrs per week
