An opportunity has arisen for a passionate and self-motivated individual to join our team as a Young People's Drug and Alcohol Practitionerbased in Flintshire**.**

Location of Work: Across Flintshire

Hours of Working : 18.5 hrs

Salary Banding: £23,352 Per annum (pro rat

Accountable To: Associate Director

Reports To: Sorted Team Manager

Job Purpose:
Flintshire Sorted provide a comprehensive service to young people across Flintshire including universal, targeted and specialist services. This post will provide targeted and specialist services to support young people at increased risk of using substances and for those identified with a substance misuse problem.

Main Duties and Responsibilities:
Client Work

▪ To undertake specific young people's assessments re substance use and related issues in respect of referred clients under the age of 25

▪ To establish, maintain and then disengage from relationships with the client group.

▪ To work with young persons in a time and task specific manner on a variety of issues likely to impact on their well-being, including their relationships, current employment, training and education status.

▪ To provide a range of interventions including education, harm reduction, motivational interviewing, relapse prevention and exit strategies as appropriate.

▪ To deliver emotional health intervention programmes.

▪ For those appropriately qualified in any alternative therapy to administer treatments as required.

▪ To care manage individual clients who have a complex set of needs and work in partnership with other agencies.

▪ To provide a specialist support and consultancy service to other professionals in respect of individual clients they may be working with on substance use related matters.

▪ To provide a range of drug and alcohol treatment and support services to vulnerable young people involved with the Youth Justice Service including cases referred by the Prevent & Deter Panel / Bureau/Court.

▪ To work in partnership with Flintshire Sorted colleagues to maintain a seamless, continuous and effective service.

▪ To provide specialist support and training for other professionals.

▪ To provide a smooth through care to adult services as appropriate.

▪ To assist in the ongoing promotion and development of the Flintshire Sorted in the local area.

▪ To ensure the health and safety of yourself and others who may be affected by the work.

▪ To provide a service that accounts for the assessment of risk.

▪ To participate in the delivery of group work with targeted groups of young people in school and community settings.

▪ To work as part of a team with other Flintshire Sorted workers and relevant agencies in developing and delivering Drugs Awareness / Prevention programmes for young people.

▪ To support and contribute to the delivery of DACs
- drug and alcohol community support initiatives involving youth related ASB.

▪ To develop learning opportunities for young people not in mainstream education in places such as Pupil Referral Units, colleges and community venues as appropriate.

▪ To raise awareness within the target group of the risks of becoming involved in substance misuse.

▪ To positively work with and support young people at risk of substance misuse.

▪ To work as part of a team in contacting hard to reach young people in a variety of settings.

General:
Administration
- To work within the Flintshire County Council / Flintshire Sorted/ADFERIAD policy framework especially in relation to the Child Protection procedures, Curriculum Development, Equal

Opportunities, Financial Regulations, Health & Safety and Staff Development Policy.
- To ensure that the Flintshire County Council Drug and Alcohol Policy and detached/outreach/mobile guidelines documents are adhered to.

▪ To maintain regular contact with ADFERIAD.

▪ To maintain contemporaneous client case notes.

▪ To complete performance data as required.

▪ To follow appropriate procedures when making referrals to other services and agencies.

▪ Prepare/contribute to the preparation of individual client and service reports.

▪ To attend ADFERIAD meetings as identified by managers.

▪ To attend informal and formal review meetings as identified

▪ To participate in delivery of drug and alcohol awareness/training sessions, where appropriate, to specifically identified groups of either clients or workers/professionals.
