**The Role**:
**The post holder will promote and work within the Discharge Lounge by**:
- Ensuring a high standard of nursing care is provided to patients from all specialties and that an acceptable standard is maintained while awaiting discharge home within the Discharge Lounge.
Liaise with transport services, social work, and pharmacy for an efficient discharge for the patients**Additional Information Specific to the Role**
- Co-ordinate between different professional groups within the hospital, and if necessary between hospitals and other external care agencies to employ best practice in discharge management.
- Share information between the hospital and external agencies
- Prescription tracking and collection on a regular basis
- Assistance in bed busting when required
- Assist the flow team when required
- Shared responsibility in the ordering and reviewing of hospital beds, mattress’s and commodes Data collection**NHS Lanarkshire**:

**What we'll need you to bring**:
- Knowledge of personal care and related procedures or previous volunteer or work experience in a health, education or social environment.
- Required to undertake on the job training and induction
- Ability to work with people and as part of a multidisciplinary team.
- Ability to carry out assigned tasks effectively in a busy environment.
**It would be great if you also have**:
- SVQ2 in health care or working towards or equivalent experience working in a care environment/role
- Previous patient contact and care in acute or equivalent setting

This role will be based in Patient Flow
- Discharge Lounge Team within University Hospital Hairmyres
The working pattern for this role is Rotational and will cover Dayshift, Backshift and Nightshift.
