An exciting opportunity has arisen for the Swansea Bay Specialist Palliative Care Team to advertise an Advanced Palliative Paramedic Practitioner (APPP) role to join our Community Palliative Care Team (CPCT).

The APPP is an experienced and further educated registered paramedic who provides holistic care for palliative patients using a medical/management model of care. Advanced practice is a level of practice, rather than a type or speciality of practice.

The role's primary area of work will be in the community setting working as part of the multidisciplinary team, however, the APPP can work in or across all clinical settings.

The post holder will provide clinical leadership, to promote high professional standards, facilitate learning, provide clinical supervision and mentorship and act as a paramedic advisor to members of the CPCT and the wider Health and Social Care Partnership.

This role will provide a rapid response to patients in the community already know to the CPCT who are in crisis. They will also see patients in an inpatient setting both in the acute hospital and palliative care unit.

The APPP will be involved with performing the initial holistic assessment of patients referred urgently to CPCT. Within the context of the presenting condition, undertake a full systematic assessment, mental health assessment, physical examination and social health assessment of patients who present with undifferentiated and undiagnosed problems.

To provide a higher level of communication skill, using negotiation and influencing skills to communicate effectively.

Contribute to the development and acquisition of palliative care paramedic practice to enhance and extend the practice of other palliative care paramedics and health care professionals within the service and beyond.

Collaborate with the multi-professional team to develop a shared vision of the service. Work collaboratively with colleagues to develop effective documentation, pathways, protocols and guidelines for care.

Contribute to the development of the team and the service to continuously improve patient care.
