We have an exciting opportunity to recruit a Support Co-ordinator to join our Young Persons team within Safety & Support. In this key role, you will be responsible for delivering a core and outreach support service that enables 16-25-year olds to obtain, maintain and sustain their tenancy and prepare them to move on to independent living. Within this key role, you will be based in our supported living scheme, Holmewood.

You will effectively manage an agreed caseload that supports residents on a one to one basis to understand their rights and responsibilities, including relevant policies, procedures and best practice.

You will support residents with issues such as substance misuse, domestic abuse and mental health services, you will assist them in accessing available specialist support. Residents’ progress will regularly be reviewed through the implementation and follow up of support plans.

To ensure continuous improvement of service, you will assist the Supported Manager in the reviewing of processes and procedures, maintaining records and completing necessary documentation in accordance with service delivery standards. You will provide effective handovers to your colleagues to ensure the high level of support to the young people is maintained.

You will also be required to liaise with key stakeholders, including internal teams and external partner agencies, to develop and maintain good working relationships and access for young people.

This is a rewarding role that delivers a sensitive service, responding to the needs of the individual whilst empowering them to live independently. You will be pivotal in supporting the young people to live well and move on to independent living, enabling you to build a career to be proud of.
