To manage a discharge caseload of clients with complex mental health needs/dementia and undertake case management/ admission and discharge CPA as needed.

To be able to communicate with and meet the needs of families and carers as appropriate to the care group and the individual client.

To have a knowledge of both organic and functional illnesses.

To receive management supervision from team manager or equivalent.

To demonstrate excellent communication skills.

To establish professional links with other care groups and professionals.

To work in partnership with other agencies to gain the best person centred outcomes for the individual.

To ensure that up to date written and electronic records and activity data are maintained in accordance with Professional and Trust standards and to provide specialist reports relevant to practice setting.

**About the role**:
As a Discharge Coordinator at West London NHS trust OPMHS you will be based across the three London boroughs of Hounslow, Hammersmith & Fulham and Ealing, comprising of: Three inpatient wards; Jubilee, Meridian and The Limes

The role’s key focus is to facilitate effective discharge from wards and departments and to make a positive contribution to patient experience and to ensure that patients are discharged appropriately and in a timely manner. The post holder will assist the Multi-Disciplinary Team, in providing a high quality service to all patients to expedite safe and timely patient discharge. Ensure patients receive relevant, comprehensive and accurate multidisciplinary assessment of their continuing care needs to facilitate safe and timely discharge from hospital.
