You will need to register if you do not already have an account Carry out venepuncture and other appropriate healthcare procedures in patients own home or in community health care settings, acting within the procedures outlined in the Phlebotomy competency framework. Undertake appropriate patient care assessments as required, such as baseline observations.

Escalate any concerns regarding patients welfare to the registered members of the Integrated Community Team in a timely manner. Once a visit is complete, enter contemporaneous notes in the relevant patient information system. Use DCHS patient information systems as needed, including accepting e-referrals. Recognise the importance of confidentiality and sensitivity of issues at all times and be able to act using the utmost discretion and confidentiality.

Additional duties and ad hoc requests may be required within this role and the need to be flexible is essential.
