We are looking for an enthusiastic individual to join our integrated Complex Assessment Team as a dynamic discharge Facilitator based at Samuel Johnson Hospital.

The Integrated Complex Assessment team are responsible for the tracking and assessment of patients admitted from acute hospitals in to Discharge to Assess beds that may require further rehabilitation or assessment in a non-acute setting.

The successful applicant will be an integral part of the team, supporting information gathering, coordinating, planning and progressing daily discharges working closely with other professionals to support patient centred outcomes

The post holder must have excellent communication skills, IT competent, be able to work well under pressure and prioritise their work load assisting in the safe and timely discharge of patients.

Some travelling may be involved.

Shifts to cover 4 days a week 8am-6pm.

This role is to cover Maternity Leave
