A permanent position is available working at Mold Community Hospital as a Band 6 Sister.

The successful applicant must have excellent communication, organisational and leadership skills and be able to work on their own initiative but also as part of a team. You will be expected to support the management of a busy ward within the community hospital setting. In addition you will be required to Deputize for the Sister in her absence making decisions and leading a professional group of nurses and a multidisciplinary team.

You will need to be proactive and flexible in your approach supporting the needs of the service.

Designated to take charge of the clinical area on a regular basis, and in the absence of the Ward Manager provide a visible and accessible resource to staff, patients and their families within the ward environment.

Responsible for the assessment of the patients nursing care needs, and the development, implementation and evaluation of programmes of care to meet those nursing needs.

Demonstrate high standards of nursing practice and provide sound clinical advice within the ward area.

Ensure that patients and carers/relatives are involved in the planning and delivery of care.

Ensure that effective communication is established and maintained with patients and carers/relatives.

Ensure patient needs are met by working collaboratively with other professionals and agencies, especially in relation to ongoing care needs and discharge arrangements.

Collect, collate, evaluate and report information, maintaining accurate and up to date patient records.

Contribute to the development of local nursing guidelines and procedures.

Recognise and respond appropriately to urgent and emergency situations.

To support Ward Manager in monitoring and sustaining high standards of cleanliness in own clinical area.

Promote and support Nurse Led/Facilitated discharge within area of responsibility and ensure all staff actively participates in discharging/transferring “medically fit” patients to more appropriate settings.
