Band 4, Virtual Services Team Coordinator

Following the exciting development of our Multispecialty Virtual Ward in conjunction with our partners within Bradford District and Craven we have an opportunity for a Virtual Services Team Coordinator within the service.

The Virtual Ward is an innovative service and the aim to is support people to receive care, support and interventions within their own home, this improves the quality of care for the individuals, reduces the length of time people stay in hospital, reduces the need for people to be admitted to hospital and ensure beds are available to treat the acute admissions.

The role will involve working as part of a multidisciplinary team alongside colleagues both within secondary care and the community.

The successful applicant will provide support to the Virtual Services Team through the co-ordination of referrals.

This post is an integral part of the multidisciplinary team which contributes to supporting patients to meet their nursing goals in their own home following an admission to hospital or a period of ill health.

- To prevent unnecessary admission to hospital
- To facilitate early discharge from hospita
- Maintain a database for all virtual ward referrals and follow each patient through until discharge.
- Analyse data and produce reports to demonstrate success of the virtual ward.
- Work closely with the Clinical Leads and referring clinicians.
- Assist with checking, recording, and passing investigation results on to the relevant clinician and document patient outcome.
- Liaise with Multi-Disciplinary Team members across all departments/wards who have patients involved with the virtual ward.
- Prioritise workload and ensure appropriate staff are aware of referrals in a timely manner.
- Support the Command Centre in providing a patient focussed quality service.
- Support the Command Centre by providing administrative and clerical support at times of need.
- Prepare reports and documentation utilising the appropriate software packages.
- Assist in the maintenance and development of the departments records management system.
- Liaise with project leads and other Trust departmental staff to facilitate the production of monthly KPI dashboard.
- Identify risks to the patient or staff, reporting the information at the earliest opportunity to a qualified member.
- Undertake phlebotomy (If holds relevant skill set).
- Promote people’s equality, diversity and rights.
- Enable people to manage disability, loss and change.
- Contribute to the protection of individuals from abuse.
