Discharge co-ordinator Band 4

33 hours per week to be worked over 5 days.

The post holder will be working in an integrated multi professional patient flow team and will be involved facilitating a safe and timely discharge for all patients on the ward. The post holder will carry-out onwards referrals to community services and liaise directly both with patients and their relatives as required about discharge arrangement.

Reporting to the Patient Flow Lead or in their absence the Admission flow coordinator or the Senior nurse on shift out of hours

The post holder will be expected to facilitate discharges over the 7 day period to meets the needs of the patients - this will include working some weekends and bank holidays on a rota system
