A position has arisen in our Long Covid Service based at Victoria Central Health Centre based in Wallasey.

To be able to work independently within a Multi-disciplinary team to deliver a Long Covid Assessment Service for the residents of Wirral.

To be able to assess, support and deliver a comprehensive rehabilitation programme for patients experiencing the long-term effects of Covid.

Work within a multi-disciplinary team assist in the co-ordination and delivery and development of a comprehensive Long Covid rehabilitation programme in line with Trust procedures.

The post holder will also manage a complex clinical caseload, assessing the patient at entry to the programme and co-ordinating their care through to discharge.

Excellent communication skills and an ability to work in a team and to direction is essential as well as the ability to work autonomously. Training will be provided as required.

To provide a multi-disciplinary approach to the management of the Long Covid within the community.

To support the delivery of NICE and other local and national guidelines and targets in relation to Long Covid.

Managing a highly complex patient caseload, including the preparation and completion of patient assessments to establish physical, psychological, and educational needs and goral setting plans including families and carers and being responsive to any communication barriers. This will also involve developing suitable care programmes depending on outcome of assessments/test results.

Lead in the deployment and organisation of long covid rehabilitation to include educational talks, exercise and wellbeing classes/strategies. To support patients to register onto the YourCovidRecovery app platform and to monitor their progress.

To be able to use own clinical judgement in the management of complex patients and the need for referral for additional tests.

Training and supervising junior staff within the department.
