Care Coordinator
**Full-time, Permanent**

Area: Gloucester Area
- **£15.00 P/H **(Depending on Experience/Qualifications)**

**We are looking for a Care Coordinator to join a successful rapidly expanding care team. The domiciliary care company is looking for someone who is caring, passionate and confident in care delivery. This role will involve care work and office work, supervisions, and spot checks. You will also be required to take part in the on-call rota. You will be working with the manager to ensure that our service is meeting and exceeding the legislative standards.**

**The Care Coordinator will be working closely with the Care Manager and Deputy Manager to provide a safe, legal and ethical practice of the day-to-day care function of the business. You will also be working alongside the Field Care Supervisor, Maintaining Excellence Lead and Support Workers - all being supported by their incredible Administration team to promote independent and quality care.**
- **To be considered for this role you will need to have a full driving licence and access to your own Car**_

Principal responsibilities:
**The successful Care Coordinator will be expected to complete tasks including but not limited to**:

- **Participate on the office duty rota and ensure effective running of all duty processes.**:

- **Reporting any major concerns to the Care Manager and taking on the role of acting up in the absence of the Deputy Manager.**:

- **Take responsibility for service care and staff compliance ensuring company policies and CQC regulations are adhered to**:

- **Rota Coordination and the allocation of visits to both service users and staff.**:

- **Dealing with urgent care needs, sickness cover, the affect on the rota and re-allocating staff accordingly during business hours and on-call.**:

- **Processing new clients referrals and taking on new care package.**:

- **Manage a database of service users and highlight to the Deputy Manager and Field Care Supervisor when tasks such as reviews, care plans and risk assessments require an update**:

- **Maintaining and updating the care management system at all times.**:

- **Ensuring all customer and Care & Support Worker documentation and electronic records are kept up to date at all times.**:

- **To ensure compliance with all office systems and procedures.**:

- **Production of reports and management information as required.**:

- **Ensure client’s care/support plans and risk assessments are followed and regularly reviewed**:

- **Support with the recruitment and retention of staff**:

- **Communicate with all relevant contacts regarding the client’s social care and well-being**:

- **Ensure Carers are fully trained and shadowing has been implemented to the required standards.**

The ideal Care Coordinator will:
**- Have experience in domiciliary or community care within a supervisory capacity**

**- Be NVQ level 3 qualified in Health and Social Care or equivalent**

**- Be confident working with service users**

**- Have a solid understanding of care planning and how these are implemented**

**- Must be willing to undertake DBS checking.**

**- Must have a full driving licence and use of vehicle for work purposes.**

**- Confident to supervise a team and managing others in a care environment.**

**- Be fully hands on to deliver care to clients and cover staff absence in an emergency.**

**- Ability to work on own initiative and be able to prioritise your duties.**

**- Good communication skills**

**- Good IT skills**

- NVQ Level 2 Health & Social Care (preferred)

Work Location: Hybrid remote in Gloucestershire
