This post is a ‘system post’, hosted by Lincolnshire Community Health Services NHS Trust with equal responsibility and accountability to all system partner organisations involved with integrated discharge. Working with all Lincolnshire system partners and beyond the post holder will take a lead role in the day-to-day development and management of a fully integrated multi-disciplinary discharge service, ensuring local and national targets are achieved.

Lincolnshire System partners have come together to redesign and improve our integrated discharge services in line with NHSE hospital discharge planning to embed a ‘HomeFirst’ and Discharge to Assess approach, meaning more people with return home from acute hospital and community hospital beds sooner and have their long-term needs assessed in their own environment.

Senior manager who can make operational decisions on behalf of the agencies, who the post is accountable to, in the system to facilitate discharges.

Work strategically to create and maintain a culture of trust and collaborative working across the system and deliver seamless working with reduced handoffs and delays.

Ensure that effective care optimising outcomes is at the centre of everything and provide strategic leadership aligned to vision to enable the continued development of the Integrated Discharge Planning function (‘The Hubs’) facilitating discharge into and across P1, P2 and P3 pathways.

LCHS recognises the increasing demand for more flexibility in working practices in order to accommodate the personal goals and commitments that employees experience at different stages during their working lives. Arrangements can be put in place if it can be shown that the needs of the individual can be balanced with those of the service and the impact on other team members, enabling the Trust to recruit and retain skilled staff, raise staff morale, reduce absenteeism and respond to changing service needs more effectively.

The post holder will ensure that effective care optimising outcomes is at the centre of everything and provide strategic leadership aligned to vision to enable the continued development of the Integrated Discharge Planning function (‘The Hubs’) facilitating discharge into and across P1, P2 and P3 pathways.

The system partners have an excellent track record of working together collaboratively under the ‘Home First Partnership’ group reporting into the overarching UEC system governance via the patient flow group. There are ambitious plans to develop the existing acute care transfer hubs to create a ‘single transfer of care approach’ to create a single but multi-site engine room managing all admissions to intermediate care capacity whether hospital discharge or step up from community and primary care to support admission avoidance onto P1, P2 and P3 pathways.
