We are recruiting a full time permanent Band 7 SEND Co-ordinator to work alongside our current SEND Co-ordinator and the Preparing for Adulthood Lead to ensure that our staff and health services meet their statutory responsibilities for children and young people (CYP) with special educational needs and disability (SEND) aged 0-25, in line with the SEND Code of Practice. This includes working across the Healthy Family Teams 0-19 and Children's Specialist Services. It is a rare and exciting opportunity to support transformation of our services with a key role in designing, developing and implementing priorities in relation to SEND.

**The post holder will**:

- Work within the Community Children & Young People's Service and Healthy Family Teams 0-19 to co-lead and develop the SEND agenda for young people, aged 0-25 with EHCP/complex physical health needs and/or life limiting conditions.
- Develop and maintain strong partnership working across children's community health services and the wider health system and ICS to design, develop and implement priorities in relation to SEND.
- To seek out new opportunities in terms of good practice and innovative research which can be adopted locally.
- Develop and ensure effective processes are in place for the identification, assessment, planning and health notification of children who may have SEND and identify any gaps in provision.
- Act as an expert resource, providing support, advice and leadership to the workforce, in managing operational issues integral to the EHCP for highly complex cases.
