The post holder will lead in the implementation and delivery of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (revised 2022) and be able to apply it within the context of assessment, review and identifying outcomes for the assessed needs of individuals, which will include assessing individuals with mental health needs and or dementia.
