We have a part time vacancy within our Therapy team working across our 3 wards in our community hospitals at Clacton and Harwich.

We are currently seeking individuals with experience working in Rehabilitation to come and work in our community hospitals, supporting qualified Therapists with day to day assessment and treatments. It is expected that the post holder will be working within a rehabilitative setting or has worked with or works with Therapists.

You will work in Clacton and if required Harwich. We provide a high standard of care and successful rehabilitation in a nurse lead environment.

We look after patients with a range of requirements including general medical, rehabilitation, palliative care and post-surgery care.

We are also commissioned for 10 stroke beds where we have KPI’s to meet locally for a seven day service.
- Enable patients and their carers to make connections with voluntary and statutory services to meet individual needs
- Assess individual needs and set realistic goals in collaboration with patients families and carers
- Develop and contribute to a shared care plan to be used within the Multi-Disciplinary Team
- To assist in managing safe and effective discharges, working effectively as a member of the multi-disciplinary team (MDT).
- To carry out Activities of Daily Living and functional assessments and report back to therapists.
- To assist in carrying out home visits and assessments in order to identify equipment needs and barriers to patient discharge.
- To monitor patient progress linking with multi-disciplinary team as appropriate.
