Commissioning Officer

Unpaid carers play a crucial role in providing essential care for people who need help because of ill-health, frailty, disability, a mental health condition or an addiction. Most unpaid carers find themselves in this role simply because they care for or love the person they support and are willing to help. Or because they cant see any choice but to provide unpaid care.

This role is to work collaboratively with our systems partners and reach out to unpaid carers whose loved one is currently in hospital or in a discharge to assess bed. To be part of multi disciplinary teams to ensure unpaid carers are being identified, supported, advised and have the right information and advice. To build strong relationships with the all age carers hub, G.P practices, community agencies and partners. Manage a small team and assist in driving the whole of the carers agenda forward.

Contribute to an support the co-ordination, the development, monitoring and implementation of commissioning plans for health and social care including public health services. to play an active role as directed in supporting the consultation and engagement in relation to commissioned service especially the all age carers agenda, including quality assurance activities

To undertake and co-ordinate multi-agency working groups and meetings on behalf of the wider team and line manager, to ensure strategic and operational objectives are delivered

To support the development of and produce financial and data analysis that informs the potential models and options including cost impact of potential/proposed commissioned services
