Main Responsibilities Lead and coordinate the operational management and delivery of the wellbeing service. Provide leadership, coordination and line management to multi-disciplinary wellbeing team inclusive of staff, volunteers and external group facilitators. Oversee the planning, delivery and facilitation of 1-1s and groups within the Wellbeing at The Hive programme; a series of informative, therapeutic and supportive 1-1s and groups for patients living with a life limiting illness and those close to them. Coordinate clinical caseload management of patients known to service, including holistic assessment, review and allocation of services.

Work alongside the Head of Community Services to identify opportunities for growth of the service, set objectives and deliver change in line with the clinical strategy. Collaborate with other clinical leads, ensuring consistent communication between services and in the development of joint working opportunities. Engage and network with external health and social care services and professionals across the locality; identifying new referral pathways and signposting opportunities for patients, whilst building relationships with stakeholders. Collate feedback from stakeholders that can inform service development and evidence safe and effective practice.

Clinical Responsibilities Complete triage, holistic assessment and review of patients on wellbeing caseload, ensuring service offer meets their current symptoms, needs and concerns. Provide person-centred palliative care, allowing patients to be involved in the planning of their care. Facilitate group and 1-1 support for management of common symptoms and challenges experienced by those living with life limiting illness, working closely with other teams from community services such as therapy, family support and dementia services for specialist input. Observe clinical changes within patients, noting changes to stage of illness; liaising with other community professionals as and when required to meet need.

Initiate conversations about end of life care and decision, supporting patients in the development of their own advanced care plans. Empower patients to make choices about their care and future, enabling them to identify what is important to them. Ensure service is culturally sensitive to meet the needs of patients across the locality Maintain accurate records, ensuring that they are clear, concise and legible, and are in accordance with the NMC Standards for Record and Record Keeping, using the EMIS database. Undertake audits of clinical practice and effectiveness to evidence quality and assurance of services.

Ensure representation from Wellbeing service at daily referrals meetings and an effective triage process of new referrals into the service. Attend Community Service Clinical Review Meeting and locality MDT meeting weekly, contributing to discussion about complex needs of patients known to service. Leadership Responsibilities Contribute to Clinical Leadership and Clinical Quality and Standards to ensure compliance with CQC standards and legislation. Lead and contribute to service development and patient experience improvement projects.

Support team members by leading daily safety huddles and handovers, regular 1-1and annual PDRs. Contribute to development of relevant documentation; e.g. Internal processes and pathways, relevant policies and service literature. Monitor KPIs and data captured within the service, working with the Head of Community Services to utilise the data in the evaluation and future planning of Wellbeing services.

To work with wider team to utilise outcome measures, feedback and input from those with lived experience to inform service offer. Organisational Responsibilities Demonstrate and display hospice values throughout work; Compassionate, Accountable, Respectful and Excellent Ensure a safe environment at all times by adhering to health and safety standards, contributing to risk assessments, and reporting all identified hazards, incidents, near misses and concerns using Vantage incident reporting system. Follow safeguarding reporting and recording processes to ensure safety of patients, visitors, staff and volunteers. To adhere to mandatory training as specified by role and work within Hospice guidance, policy and procedure.
