**The Role**

We are looking for a committed, career focused individual within the Domiciliary Care market as a Care-Coordinator, covering the Chesterfield area.

You will most certainly have knowledge and insight into the Domiciliary Care market and have experience in the Care
- Coordinating role.

**Principle Responsibilities will include**:
Delivering care out in the community.

To assist the Manager in all aspects of care management.

Contribute to the development and growth, whilst ensuring SureCare’s commitment to quality at all times.

Contribute to the research, sales and development of new areas of services provision.

Actively promote and sell the services of SureCare.

To assist in the recruitment of Care Workers observing current SureCare policies and procedures.

Maintain accurate records in line with SureCare’s policies and procedures relating to the handling, storage and if necessary deletion, of personal details to ensure compliance with the General Data Protection regulations for Care Workers and Service Users. keep all records updated using computer and manual systems and implementing any security measures to protect the personal data.

Produce accurate care worker rotas on a weekly basis, ensuring all calls are covered

Observe Service User and Care Worker confidentiality.

Accurately and appropriately match Care Workers to Service Users.

Communicate and liaise with all office personnel, social workers, Service Users and Care Workers etc.

Assist with “on call” duties at weekends, weekday nights on a rota basis. (With an additional "On-Call" Rate) This may include covering sickness in the event it is not possible to cover with staff.

Deputise in the Manager’s absence.

MUST have at least 2 years experience in the Care
- Coordinator role

**Salary**: £22,000.00-£25,000.00 per year
