**Job summary**

Pontefract & Knottingley PCN is looking to recruit an additional Health & Wellbeing Coach to join our existing team.

Our PCN is made up of four GP Practices; Ashgrove Medical Centre, Friarwood Medical Centre, Northgate Medical Centre, and Stuart Road Medical Centre in the east of the district located across Pontefract and Knottingley and serving a population of approximately 49,000 registered patients.

This role will involve working across all four of these practices.

**Main duties of the job**

Health and Wellbeing Coaches are an integral part of the general practice team working in each practice across the PCN. The post holders will work in partnership with their clinical and non-clinical colleagues, management support and the wider PCN to ensure the role delivers the best possible outcomes for our patients. Health and Wellbeing Coaches support people develop their knowledge, skills, and confidence in managing their health and care, to improve their health outcomes and quality of life. Health coaches do this by using coaching techniques and motivating patients through multiple sessions to identify their needs, set goals, and supporting patients to achieve their personalised health and care plan objectives.

Health and Wellbeing Coaches may work with patients over the phone, by video conference, or face-to-face. The role however is varied, and may include supporting self-management education, peer support, caseload management and facilitating group consultations. Identify opportunities and develop and deliver project recommendations for services and actions needed for active lifestyles that contribute to health outcomes, community safety and independent living in the Primary Care Network.

Health and Wellbeing Coaches will have access to ongoing supervision, skills development, and support so they are able to further build their skills and experience within the role.

There may be a need to work remotely depending on the requirements of the role.

Conexus Healthcare Limited is the Confederation for general practice in Wakefield, we have four areas of work.
- 1. Delivery of high-quality public facing Health and Wellbeing services across the Wakefield District:_ We develop, manage, and deliver health and care services that support the population of the whole Wakefield district
- 2. Delivery of Training, Development and Consultancy support across the United Kingdom:_ We design and deliver a range of training courses, development activity and provide advice and guidance that supports general practices and other primary care organisations.
- 3. Supporting Our Practices and Primary Care Networks (PCNs): _We support general practices and PCNs across the Wakefield district to be more resilient, sustainable and deliver better patient care and care for the health needs of their populations.
- 4. Providing a voice and influence for General Practice in the Health and Care System: _We provide a forum to develop a unified voice for General Practice and support general practices to cooperate and work collaboratively with the wider health and care sector partners in the district.

**Experience**

**Essential**
- Experience of using coaching approaches/frameworks and models or other helping strategies e.g., Motivational Interviewing
- Experience of working with groups
- An understanding of the biopsychosocial model of health and the social determinants of health.

**Desirable**
- Experience in health and nutrition work with patients/clients.
- An appreciation of the new NHS landscape including the relationships between individual practices, PCNs and the commissioners
- Networking experience
