The Therapy assistant will work as part of the Nursing workforce and therapy team to deliver high quality, person centred, compassionate care to patients.

They will be part of the multi-disciplinary team delivering planned care to a group of patients working within the standards of proficiency outlined by the in accordance with Trust policies and protocols.

In collaboration with the patient, plan and organise interventions specific to their individual needs and within the remit of their care plan
- To contribute to care planning and the review process for a caseload of patients.
- To provide direct personal care to patients using initiative in response to patient need
- To work in conjunction with qualified practitioners in order to implement care planned interventions and treatments focusing on the improvement on the needs of patient.
- To support and plan care alongside the therapy team on the ward
- Work as a supervised primary nurse planning and implementing care to patients
- Effectively co-ordinate patient care as necessary on a shift by shift/day by day basis and document care in patient notes.
- With the support of the qualified health or social care professional; plan, organise and implement tasks, activities and care programmes for identified patients.We are an award winning and CQC rated good, health and social care Trust that delivers care to communities in Hull, East Yorkshire, Scarborough and Ryedale and Whitby.
