** Community Registered Nursing Associate**

Our Attleborough community nursing team has a rare opportunity for a Community Nursing Associate

The Ketts Community Nursing Team is part of Norfolk Community Health and Care NHS Trust and part of the South Place working within and surrounding villages. The job predominantly requires visiting patients within there homes and care homes.

The job requires you to contribute to the delivery of care packages for patients with long term conditions, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible. To work within the integrated team to facilitate early discharge from hospital To work within the integrated team to prevent unnecessary admission to hospital. To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.

The successful applicant will have a sound clinical background with a high level of skills and knowledge

Maintain appropriate patient and clinical records in line with the Trusts policies and procedures, and in line with the agreed service specification. In line with the annual personal development plan the post holder will be expected to undertake any training or development required to fulfil their role. The post holder must carry out their responsibilities with due regard to the Trust’s Equality and Diversity Policies, Procedures and Schemes and must ensure that equality is promoted at all times.

The post holder will be responsible for your own daily caseload with supervision and support from the Clinical Lead and the Band 5 & 6 Nurses.

You will be expected to have excellent communication skills & IT skills and be able to provide evidence of working successfully at Band 3 and above.

The post holder will need to show there experience of contributing to assessments and care planning and carrying out interventions to contribute to patient health and wellbeing.

The post holder will have access to training and supervision that will build upon their skills and will help them to progress as a competent band 4.

The post holder will need to be a car driver with a full current driving licence.
