**About the Trust**:
The South London & Maudsley NHS Trust provides a full range of mental health services to people of Southwark, Lewisham, Lambeth, and Croydon as well as substance misuse services for people in Bexley, Greenwich, and Bromley. The Trust provides specialist services to people from across the UK.
It has approximately 4,800 staff and a budget of £300 million. The trust also works closely with the Institute of Psychiatry, Psychology and Neuroscience (IoPPN), King's College London and are part of King's Health Partners Academic Health Sciences Centre. There are very few organisations in the world that have such wide-ranging capabilities working with mental illness. Their scope is unique because it is built on three major foundations: care and treatment, science and research, and training.
The Assertive Outreach Team is part of the larger AO & Rehabilitation CMHT focused on providing intensive support for patients with complex mental health and routinely with under-lying physical health needs including dual diagnosis in the community. We work collaboratively with placement providers and other Allied services to offer a seamlessly coordinated and robust community support service focusing on people’s strengths to support their whole health and wellbeing.
- Previous experience of Care Coordination is desirable

**Responsibilities**:
As a care co-ordinator within our team there are many opportunities for personal development, and learning opportunities around social care, physical health care, and MDT working, with an opportunity to take part in depot and physical health clinics.
The role includes leading the delivery of comprehensive service user assessment, planning, implementation, and evaluation of person-centred care in a collaborative way. The post holder will co-ordinate a first-class service that includes assessment and monitoring of physical health, maintaining service user’s rights, working collaboratively with the Multi-Disciplinary Team and maintaining a safe and therapeutic working environment and accurate records.
