**We have an exciting opportunity for an experienced Link Worker / Social Prescriber to undertake a new role within the North Gosforth Primary Care Network.**

The focus will be on patients who do not have alternative forms of support and will not duplicate the work of existing provision locally. Referrals into our service will be identified by the practices and it is envisaged this will support older people to live independently in the community as we will work with them to get the support that is most meaningful to them.
