The Community Rehabilitation Enhanced Support Team (CREST) is a regional community mental health service to help people live outside of more complex locked rehabilitation across the West Yorkshire region.
We have two vacancies available for the CREST team. There is a position available for the Serious Mental Illness pathway and one for Complex Emotional Needs.
You will work with the established Occupational Therapy Team to provide Occupational Therapy input to service users and carers in relation to engagement work, functional skills, activities of daily living, routine, roles & integration to the community. We promote the service user voice, with the promotion of collaborative goal-setting.
Occupational Therapy is an integral part of our multi-disciplinary team which also includes, Mental Health Nurses, Psychologists, Psychiatrists, Physiotherapists, peer support recovery workers. We work closely with the ASCEND model, ensuring recovery and hope is integral with our practice.
You will be a valued member of our highly supportive multi-disciplinary, community service and AHP community. You will manage an identified caseload of clients with a range of clinical need including engagement, functional skills and at times, equipment. You will provide a range of occupational therapy interventions that are guided by the registered occupational therapist. You will be responsible for feeding this back in MDT, documenting and supporting service users with their care plans. Alongside working with a wide range of social care partners.
You will be offered supervision from within the Occupational Therapy Team, receive regular training and development opportunities including training in interpreting OT assessments, goal-setting and graded interventions.

To promote a positive image of the occupational therapy and community services of Leeds and York Partnerships NHS Foundation Trust at all times.
