Delivering One-to-one Service advice and support out in the Northumberland community:

- Providing knowledgeable, compassionate, and trusted advice, guidance and support to people affected by dementia.
- Helping people understand dementia and how to cope with day to day living whilst preparing for the future.
- Assessing the needs of people affected by dementia.
- You will also assist service users to access other services, providing signposting and referrals.
- Empowering individuals living with dementia to maintain their independence, improving their sense of well-being, and putting them in more control of their lives, making informed choices about their own life.
- Networking and building meaningful relationships with a wide range of local organisations, promoting Alzheimer’s Society services to help people with a diagnosis to get referred to the service as early as possible.
- Working closely with Northumberland Memory Service and Primary Care Networks across the county.

**About you**
- An ability to assess and evaluate client needs along with the understanding of the need for client confidentially is essential to providing a valuable and worthwhile service.
- Excellent person-centred and non-judgmental communication approach and active listening skills.
- Great organisational skills to manage your caseload of clients effectively.
- It is desirable you have experience of coordinating others such as volunteers.
- Passionate about to putting people affected by dementia at the heart of everything you do, to make the service our best it can be.
- Great networking and influencing skills.
- Experience of developing relationships and liaising with statutory and voluntary agencies.
- Good IT skills and the ability to maintain records and write reports.
- Be able to travel independently across Northumberland.
- An understanding of dementia and the needs of those living with dementia and their carers.
- Be able to represent the needs of the organisation externally.
