Job description

**JOB PURPOSE**

To deliver a highly effective 24 hour Step-Down wrap around Supported Service for semi-independent living providing quality support for young people aged 16 years and up to 18 years+ (and including those with a disability up to the age of 21 years) which meets the National Minimum Standards. To ensure that all aspects of physical and emotional support of the young people are met so as to promote their welfare, whilst working in partnership with their families and other professionals.

To provide experience and expertise in working with young people and families and to effectively support manager and staff team. To enable Young People to have maximum control over all aspects of their daily life and to ensure they are supported to develop their abilities in line with their Pathway Plans and risk assessment. To comply with Greater Horizons Policies and Procedures.

**MAJOR ACCOUNTABILITY AREAS**

1. To support and engage in work with young people, individually and in groups to achieve optimal outcomes, including preparation for adult life programme, appointments, meetings.

2. To support the effective delivery of Greater Horizons Service Aims.

3. To effectively and efficiently support the use of financial resources within the home.

4. To effectively support the use of physical and human resources within the home.

5. To develop productive working relationships with colleagues, other agencies and members of the public.

6. To contribute to project planning and preparation.

**MAIN DUTIES & RESPONSIBILITIES**

1. Induct Young Person on admission, explaining house rules and welcome pack etc.

2. Continually assess Young People's needs and work with Senior Support Worker / Manager / Service Manager to revise support plans in line with Young Person's changing needs and abilities.

3. Provide a positive, welcoming and stimulating environment for Young People, leading by example in encouraging them to carry out day to day duties.

4. Be a point of contact for Young People to feel able to discuss ideas and keep them updated on actions.

5. With the Young People agreement, act on their behalf when appropriate.

6. Promote, acknowledge and respect the privacy, dignity, diversity, rights and responsibilities of Young People to enable them to meet their goals and aspirations and to build and maintain their self-worth and individuality.

7. Liaise with Senior Support Worker / Manager to make recommendations on how support can be adjusted to promote independence.

8. Identify development areas in Young People to enable their independence and be innovative in coming up with methods to support these.

11. Respect and enforce professional boundaries and communicate professionally with Young People, colleagues, professionals and agencies.

12. Ensure general up keep of the house is to a high standard by reporting maintenance issues, and health and safety issues.

13. Implement support plans and ensure completion of key documents and Young Peoples files.

14. Communicate effectively with staff and Young People to ensure activities are happening to deliver the support plans, ensuring staff and Young People are aware of the information needed to complete tasks successfully.

15. Produce daily reports and correspondence to a high professional standard.

16. Manage the accommodation, ensuring that health and safety requirements are met at all times.

17. To undertake household duties when required in order that a safe, clean and comfortable environment is maintained.

18. Monitor expenditure against budget provision for support and advise on money management where required.

19. Provide weekly and or monthly progress reports.

20. Respond to informal complaints made by or on behalf of Young People.

21. To attend, as necessary, PEP, EHCP, LAC Reviews of the Young People.

22. Liaise with professionals / carers as necessary and attend professional meetings as required.

23. Ensure concerns and reasons for young people are properly identified and logged using the correct forms and communicated effectively to relevant parties.

25. Attend all staff meetings, training courses, supervisions.

26. Identify and request relevant training and development through supervisions.

27. Cover any gaps on shift or provide a second person on duty, until relief staff can be secured.

28. Ensure staff room privacy is protected and to not allow Young People into the staff room for any reason.

29. Assist in the day to day management of petty cash system, ensure compliance with the legal requirements relating to financial regulations, policies and procedures and implementation of quality assurance systems for which, the post holder is responsible.

30. Develop and sustain effective working relationships between workers and the local community, colleagues and other agencies to promote collaborative working and partnerships.
