Care Coordinator; Administrative safeguarding duties To oversee the workflow of safeguarding reports and documents and ensure they are circulated to appropriate clinicians in line with practice safeguarding policies. To summarise and code documents into patient records, highlighting anything specific to the relevant named GP in line with practice policies. To maintain a register of children on Child Protection Plans, Child in Need plans and those children deemed as cause for concern. To ensure the correct coding and alerts are added to patient records to aid clinicians in identifying those children and adults who are cause for concern.
To liaise with patients, clinical colleagues and other stakeholders to ensure that the most up to date contact, demographic, and household details are held on record. Partner Communications To respond to child protection meetings as advised by practice safeguarding lead, preparing appropriate documentation and sending apologies as necessary. To monitor DNAs for children in regard to missed clinical appointments and support clinical colleagues (e.g. Practice Nurse, usual GP, Safeguarding Lead) to follow up with patients in line with practice safeguarding policies.
To follow up on those children who have not had their routine immunisation, as identified by administration with the appropriate clinician (e.g. Practice Nurse) in line with practice safeguarding policies. To support in co-ordinating communication and meetings with third parties e.g. Health Visitors, Safeguarding Leads and Social Workers.
This includes liaising with all stakeholders, meeting scheduling, document distribution, taking minutes and updating the patient record in accordance with agreed actions. Expert Practice and Practice Development Be a role model for all clinical staff by practicing safeguarding to the highest standard, whilst exercising a high degree of personal and professional autonomy. Be aware of local resources and disseminate updates to colleagues. To carry out case reviews and disseminate learning to colleagues across the PCN.
Demonstrate commitment to developing own expertise to support robust systems, policies, professional guidance, and training so that staff across the PCN can identify and contribute to safeguarding issues. To have regular safeguarding supervision and case reviews Administrative Vaccination and Immunisation duties Proactively ensure that eligible patients receive their immunisations and vaccinations in a timely fashion. Ensure eligible patients are contacted through appropriate means of communication to ensure good uptake and attendance. Help patients to manage their needs through answering queries and ensuring that people have good quality written or verbal information to help them make choices about their care.
Assist and potentially lead on the digital appointment booking system for vaccinations. Carry out searches on clinical systems to identify eligible patients. Invite eligible cohorts through appropriate means i.e. text / call / letter on a regular basis.
Follow up DNA appointments of immunisations. Promote immunisations to eligible patients to encourage uptake. Working with other members of the PCN team, seek to innovate vaccination delivery for hard-to-reach groups and monitor uptake across the PCN for specific groups. Other Duties Support the practices in your PCN to improving local uptake of National Cancer Screening Programmes, understand the practices data for uptake on screening and understand how the practice can carry out the screening.
Support the PCN with audits and data collection with a focus on early cancer diagnosis. Working with other PCN Care Coordinators, raise awareness of health promotion, screening, NHS Health Checks, LD Health checks and proactive and preventative health and wellbeing offered by the PCN. Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals; helping to ensure patients receive a joined-up service and the most appropriate support. Support the coordination and delivery of multidisciplinary teams within the PCN.
To contribute to data collection and audit across the PCN. Receptionist/Administrator; Identify and book appointments and home visits in line with practice protocols, including cross-organisation bookings. Search for, register if necessary and book onto the computer system patients as they present at the desk. Deal with enquiries, signposting and requests, either face to face or over the telephone from a variety of external sources, such as patients, relatives, clinical team, hospital, pathology, and work colleagues.
