Do you have a passion for working with Children and Young People? We are looking for a healthcare support worker to join our physical health pathway to ensure safe, timely and effective monitoring of the physical health needs of young people open to our service.

We have a dedicated and passionate staff team, who will provide you with a robust training package, to ensure the delivery of safe and effective care to our service users.

The Specialist Young People and Families Mental Health Service respond quickly to those children and young people referred with urgent and high-risk mental health needs, and provide collaborative, effective, evidence based interventions at the right time with the right person. This team works within a multi-disciplinary framework, and closely with other pathways and colleagues to ensure smooth transitions take place, which includes transitions to adult services or alternative specialist providers.

You will be responsible for, the delivery of physical health monitoring to children and young people, this will include venepuncture and ECG monitoring, monitoring of blood pressure, height, weight and temperature.

Part of your role will also include the promotion of healthy living to young people and their parents/carers.

You will be responsible for ensuring effective communication with professionals/the wider MDT who are involved in the care of young people, to ensure the delivery of holistic care.

1. Communicates with service-users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology.
2. Regularly reports and effectively communicates service-users health status and care aims in care records and verbal reports to allocated workers and other members of the multi-disciplinary team.
3. Relates appropriately with service-users, carers, colleagues and others - at all times maintaining professional boundaries.
4. At all times ensures that own actions, support the equality, diversity, rights and responsibilities of individuals.
5. Identifies, minimizes and reports risks of danger, harm and abuse to senior colleagues.
6. At all times minimizes risks to self by undertaking safe working practices whilst regularly working under remote supervision in community settings.
7. At all times works within agreed and established Trust policy and operational procedures.
8. Works with other Trust services to promote seamless service-user focused care, transfer and discharge on identified care pathways including accepting routine referrals during the day.
9. Responsible for contribution towards planning and undertaking delivery of planned physical health care observations.
10. Regularly provides advice and guidance - promoting healthy activities and lifestyle choices.
11. At all times takes responsibility for own personal and professional development and supports the development of colleagues, students, trainees and volunteers.
12. Regularly accesses and makes use of supervision.
13. Regularly undertakes activities to (and makes suggestions about how to) improve services.
