The role requires extensive knowledge and skills in best practice in this field of work and requires effective and direct close working with children, young people and adults (aged 0-25) and their families / key support networks. Providing advice,
guidance and practical support that is impactful and long term, and underpinned by evidenced
based practice and research.

The Family Wellbeing case practitioner will take on the role of lead professional, engaging intensively with a caseload of 12-15 families who have multiple and complex needs and leading /coordinating support services across a multi-agency network of professionals within both children and adult services with the aim of reducing the full range of risk factors that may lead to developmental delays, statutory social care involvement, high risk behaviour, non-attendance at school for children and other vulnerabilities as described in the eligibility criteria for access to the service.

Workers will broker and lead on supporting adults on the path to employment, overcome barriers to caring for and protecting their children (e.g. substance misuse, mental health needs, domestic abuse, offending, housing needs etc.) coordinating support across the multi-agency network (children’s and adult services). The role requires the practitioner to work within the Merton Early Help Practice Model and to be skilled to a high level in Relationship Based Practice and a number of other evidence-based approaches to underpin their engagement and work with families.

This role is for the ME11 grade. Please be aware that you will be expected to evidence your ability to meet the additional duties at the bottom of the JD.
