Maintaining a proactive approach to managing patient pathways and flow Supporting the head of pharmacy and clinical services in the development, implementation and compliance with PGD medication Monitoring of policies, guidelines, protocols and pathways relevant to patients with a focus on quality, safety and patient experience Supporting mobilisation of DDHF community based primary care and GP services in accordance with contract specifications and service commissioners requirements Ensuring the delivery of DDHF community based primary care and GP services in accordance with contract specifications and service commissioners requirements Support and adhere to DDHF HR management systems and processes for and by clinical personnel Support and adherence to DDHF performance and quality management systems and processes by clinical personnel Ensuring appropriate clinical governance systems and processes are developed, implemented and maintained for all DDHF clinical and clinically led services Assisting in the management and mitigation of organisational risk Liaison with external partner organisations and stakeholders, including Member Practices
