**Salary**:£29,503

**Contract**:Fixed term 2 year contract (including 6mth probation), full-time (37.5 hours), 52 weeks with 25 days annual leave, NEST pension

**Position reports to**:Feltham & Bedfont SPLW Team Leader/PCN Clinical Director/CEO
- This role will be managed by the Reach Foundation but seconded to the Feltham & Bedfont Primary Care Network (PCN) Social Prescribing Team._

**Introduction to the Reach Foundation**

The Reach Foundation is a charitable organisation providing cradle-to-career support to families in order to ensure every child can enjoy a life of choice and opportunity. The Foundation’s activities consist of:

- Reach Academy Feltham, an Outstanding all-through school educating 900 pupils aged 2-18
- The Reach Children’s Hub, providing cradle-to-career support and services to parents and their children (aged 0-21+)
- The Feltham Convening Partnership, a collective impact initiative to improve outcomes for children and young people (CYP) across the community
- Reach Training, a series of networks, opportunities and resources to share our learning more widely across the UK.

**Job purpose**

This is an exciting opportunity to join the Reach Foundation’s team as a Social Prescribing Link Worker, with a specialist focus on supporting children and young people aged 3-18. You will be embedded in the Reach

Foundation, which has over a decade of experience supporting children and young people in the local area, whilst at the same time contributing to and benefitting from the experience of the wider social prescribing team employed by Feltham & Bedfont PCN.

**Outcomes**
- Work in collaboration with the Primary Care Division of Hounslow Consortium Limited and the Reach
Foundation, to deliver a coordinated and high-quality Social Prescribing Link Worker (SPLW) service. This role will specialise in supporting children, young people and their families/carers to access and engage with the extensive range of support in the community. They will be employed, trained and supported by
Reach Foundation and seconded to Feltham & Bedfont PCN.
- Use social prescribing to empower children, young people and their families/carers and to take control of their health and wellbeing. They will spend time with clients to help them to focus on ‘what matters to me’ and connect them to community groups and statutory services for practical and emotional support.
The role is not intended to support individuals and families long term, but to help them to understand how they can support themselves better.
- Manage a caseload of clients each year through assessment to onward referral, working with clients in the
PCN GP practices and community. The post holder will provide ongoing support for an allocated but short-term timeframe to promote engagement with identified services and achievement of goals.
- Contribute to the development of the service and participate in support, supervision and training as required. The role is a non-clinical role; however the post holder will receive support from both the employing organisation and also a clinical mentor in General Practice.
- Contribute to the education of GP practice staff within the network and maintain details and grow working knowledge of sources of support in the community.

**Key responsibilities**

As a specialist Social Prescribing Link Worker embedded in the community, you will be uniquely positioned to build relationships with a wide range of stakeholders, including local GP surgeries, other voluntary sector organisations and schools. These relationships will support and enable you to successfully uphold the responsibilities of this role, which include:

- Provide personalised support to children and young people, their families/carers to take control of their wellbeing and improve their health outcomes.
- Signpost, refer and supportively bridge Children and Young people into appropriate services and partner agency support, ensuring follow up of referrals takes place.
- Build positive, trusting relationships with young people that support them to engage in appropriate services and specialist interventions, and to remain as a consistent point of contact throughout their social prescription.
- Build relationships with key staff within schools, attending relevant meetings, becoming part of the wider team, giving information and feedback on social prescribing.
