To work within the community, as an autonomous, accountable Paramedic and provide holistic care for individuals including assessment, diagnosis and management. Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the team. To refer and/or signpost patients in their own home or care home to other allied health professionals for example community COPD nurses, Social prescribers etc. To be able to;
- perform specialist health checks and reviews - perform and interpret ECGs - perform investigatory procedures as required - undertake the collection of pathological specimens including intravenous blood samples, swabs, and other samples within their scope of practice, and within line of local and national guidance; Support the delivery of anticipatory care plans and lead certain community services (e.g.

monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing). Provide an alternative model to urgent and same day GP home visits for the network and clinical audits. Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice. Advise patients on maintaining health and refer to allied services when a health promotion need is identified.

Communicate at all levels within the multidisciplinary team ensuring an effective service is delivered. Communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings, and treatment choices. Take responsibility for safeguarding children and vulnerable adults and work with other team members to ensure they are protected. Work closely with the multi-professional, management and administration teams, reporting any problems encountered to the relevant person and ensuring everyone is aware of the different roles within the team.

Work closely with the wider PCN and Community teams. It will be necessary to attend and contribute to various MDT meetings as requested.
