An exciting opportunity has arisen to join the IntermediateCare Services and Specialist Medicine Management Team at Glan Clwyd Hospital

The Patient Pathway tracker role includes management of the patient journey through the care pathway in accordance with Welsh Government Referral to Treatment (RTT) guidelines and requirements.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

The role of the Pathway Tracker is to provide comprehensive administrative support to the Area Team by collecting, recording, analysing, interpreting and reporting of data required to monitor compliance with the Welsh Assembly Government Referral to Treatment Time (RTT) and Service and Financial Framework annual targets.

Responsible for accurate and timely recording of the whole patient pathway on WPAS.
