**Hours: 37.5 hrs per week**

**Salary: £32,319 - £35,230 (Salary 30-33)**

**Contract: Permanent**

**About the Role**

Aberdeen Links aims to reduce pressures on mainstream primary and community care services by meeting a need for joined up support across the Health and Social Care Partnership.

As a Senior Primary Care Link Practitioner, you will provide management, support and leadership to the Primary Care Link Practitioners, to ensure person-led support is provided in response to the needs and interests of people attending General Practice’s, supporting them to identify and manage issues that affect their ability to live well.
