An exciting opportunity has arisen for seven new discharge co-ordinators to join our integrated discharge team working with colleagues from both health & social care.

The role of the Discharge Co-Ordinator will be an integral part of the integrated discharge team who will be responsible for assisting the multi-agency teams with safe and timely discharges from the acute hospital setting. You will attend and feed information into enhanced discharge meetings, liaise with patients, carers and their families and where applicable attend patient panel meetings.
