Children & Young People’s Mental Health Transforming Care Case Worker - Learning Disabilities and Autism

We are looking for a dynamic, self-motivated and passionate Children and Young People’s Mental Health Transforming Care Case Worker FOR Learning Disabilities and Autism to work alongside senior managers, nurses, case managers and service users to help deliver our collective ambitions and aims for CAMHS services across North Central and East London.

The post holder will be part of the Patient Flow team, which is responsible for the clinical quality and clinical performance of all NCEL providers and for all patients who originate from NCE London regardless of which provider the placement is with.

As the Transforming Care Case worker, the post holder will work as part of a dynamic team in delivering an effective service to ensure safe and effective care pathways are in place for all service users from NCEL in specialised mental health care settings, within the NHS and Independent sectors.

The post holder is a critical part of the NCEL quality assurance of NHS and independent providers through constant formal and informal quality checks. It is expected that the case manager will be embedded within the provider attending governance and some operational meetings and is there to address day to day quality concerns that arise preventing them from developing into serious quality issues.

Working effectively alongside the provider is essential as this role should be a valuable resource for both parties. The post holder will need to be able to hold providers to account but do this in a supportive, collaborative manner but also know when to escalate persistent or more serious quality concerns.

The North Central and East London CAMHS Provider Collaborative is a partnership between East London NHS Foundation Trust (Lead Provider), Tavistock and Portman NHS Foundation Trust, Whittington Health NHS Trust, Barnet Enfield and Haringey NHS Trust, and North East London NHS Foundation Trust, and working in collaboration with Ellern Mede, an Independent Sector Provider. The post holder will be employed by the Lead Provider, ELFT.

The Collaborative is a provider led partnership that commissions all inpatient CAMHS services (including General Adolescent, Adolescent Psychiatric Intensive Care, Eating Disorder, Low secure and Learning Disability & Autistic Spectrum Condition) for young people living in North Central and East London.

The post holder will need to provide clinical support guided by the senior team in relation to the defined population, this will include projected mapping of future service use, identification of users for future service provision, planning in terms of future service needs.

The post holder might be required to provide cover for other case managers, including those covering over provider collaboratives, for short periods when planned or unplanned absence occurs.
