Job Introduction
The Family Solutions team is a committed team based in Telford and Wrekin’s Outstanding Children services.

Family solutions offer targeted, short term, intensive intervention and the service is made up of Edge of Care/ Stepdown / Reunification support and the Family Group Conference service.

Telford’s Edge of Care service prevents young people at risk of entering care to stay with their families by, providing intensive support to young people and their families, usually over a period of 6-12 weeks.

Our step-down support helps young people make the transition into foster carer or reunify young people back into the care of their birth family using evidence-based models of practice.

Family solutions is also made up of the family group conference service. A family group conference is a family-led meeting in which the family and friends network come together to make a plan for a child. The process is supported by an independent FGC facilitator who helps the family prepare for the family group conference.

We are looking for an individual who is passionate and committed to achieving long term sustainable outcomes for children, young people and their families.

The role is:

- To work as a member of a team providing a flexible crisis response service
- To manage a complex caseload allocated by the Team Manager or Senior.
- Providing targeted interventions to families with, intensive, support at a frequency assessed to meet the needs of the individual and/or family that will enable them to make positive changes and facilitate independence.
- To provide a broad range of interventions, that are evidenced based and delivered within local communities building strength and confidence for children and families.
- Meeting the needs of families that would otherwise result in the young person being accommodated.
- To work with children and their families/carers directly in the home and community often in crises led situations to respond to need associated with family led matters.
- Working in partnership with a range of professionals to promote the welfare of children.
- The service also safely returns children to birth families if it is safe to do so and works alongside other teams to enable a child to have a safe family experience whilst in care of the local authority.
- The post holder will contribute and undertake plans and assessments.
- Work systemically with a strength-based approach.
- To produce assessments and/or evidence-based support plans.
- The aim of the service is to promote children living safely with their families, using creative thinking and a range of tools to improve the outcomes for children and their families.
- Essential car users allowance.

The role will directly work with children and families on a planned basis or crises driven situations in the community, in their homes or local authority provision. The role requires the post holder to have extensive knowledge of working with children and families, child development, systemic practice and experience of working with other professionals in the community. The post holder must have the ability to provide a flexible approach in providing a service. This means being in a position to work planned out of hours and weekend working if required.
