**Background**

At the request of Hertfordshire County Council and both Hertfordshire CCGs, seven voluntary organisations have formed a partnership to develop a service bringing together Hertfordshire’s current community navigation and hospital discharge services.

The vision is to create a Herts-wide Integrated Hospital Discharge and Community Navigation Service (IHDCNS), with teams based locally that will understand their local populations, and support people to find and access community-based support.

The outcome we want to achieve is to improve the health and wellbeing of people who might otherwise use statutory services, maintain their financial stability and in turn manage down demand on statutory services. We want to reduce the number of individuals entering crisis (defined as unnecessary hospital admission or use of homecare or multiple contact with different agencies) and particularly to reduce unnecessary hospital admissions.

This is a service designed to bring community navigation and hospital discharge together within an integrated service with the following aims:
1. improving health and wellbeing.

2. reducing hospital admissions;
3. reducing GP visits;
4. reducing reliance on home care providers;
5. reducing social isolation;
Each Locality Manager will report to one of the Area Managers and will in turn supervise a local team of Link Workers (who may be employed and line managed within several different partner organisations). This post will also require travel within the locality and occasionally throughout Hertfordshire. The post will also require some weekend work.

**Overall purpose of the role**

Work with the Area Manager to establish an effective local service for Hospital Discharge and Community Navigation including social prescribing. Establish effective partnerships with the local voluntary sector, NHS providers and adult care services. Ensure that the service puts people first, empowers people to take appropriate decisions about their care, and to access support and services within their own community:

- Support and supervise the locality’s staff team
- Be the local “eyes and ears” of the integrated service
- Deliver a high quality responsive service
- Make sure that cases are properly reviewed and recorded
- Contribute to the development and achievement of targets and standards
- Provide appropriate and timely support to clients, carers or family members.

**Support and supervise the locality’s staff team**

1. Help the locality team to co-ordinate their work

2. Ensure that existing skills and specialisms are nurtured and shared within the local team

3. As necessary, assist with the recruitment and induction of new posts to complete the local team

4. Supervise local staff and manage their work

5. Establish a positive and professional culture of mutual support, learning and public service

6. Ensure that all staff understand the service’s targets, standards and requirements

7. Hold regular team meetings and regular 1-to-1 supervision meetings, and record each meeting

8. On HR, employment and other matters, liaise as required with the employing partner organisations

9. Recruit and develop arrangements for supervision of volunteers to assist with the delivery of the service

**Be the local “eyes and ears” of the integrated service**

10. Establish close and functioning relationships acute hospitals, out of county hospitals, community hospitals, voluntary organisations and other health and community stakeholders and possible partners

11. Capture local intelligence and share within the locality

12. Understand local needs and identify local partnership opportunities and gaps in service provision

13. Establish local relationships and informal networks

14. Raise local awareness of HertsHelp and ensure that all parts of the network understand how to make a referral

15. Make sure local needs and views are known to the Area Manager

**Deliver a high quality responsive service**

16. Receive all referrals and ensure they are effectively assessed and prioritised

17. Ensure that service users and carers are engaged in decisions

18. Ensure that staff are allocated to cases appropriately and collaborate as required

19. Ensure that required benchmarking information is systematically collected

20. Ensure that all referrals are dealt with professionally, promptly and according to protocols

21. Ensure effective liaison and communication with clients, carers, and professionals

22. Undertake frontline case work

**Make sure that cases are properly reviewed and recorded**

23. Ensure staff complete case notes and record information promptly and to the required standard

24. Systematically review cases and ensure that learning is shared effectively

25. Receive and analyse local management reports
