The post-holder is expected to be experienced in implementing change in primary care, with digital experience. The post-holder will help Groves Health Practices (the Practices) and the Primary Care Networks (PCNs) adopt new digital technology and other operational initiatives to improve access to care; enable practices and PCN staff to work more effectively; and enable patients to self-serve.

The post holder will work with the Executive Management Board, Operational Management Team and Group/Practice Managers in the Practices, and the PCN Digital and Transformation Lead, Quality Improvement Lead and leadership within each PCN to supporting service improvements; identify opportunities for positive transformation; and deliver changes in a collaborative way.
- This role will include delivery of a combination of the following responsibilities:

- Improve adoption and/or optimisation of existing technology to enhance patient access and experience and increase PCN productivity.
- Build relationships and facilitate collaboration between the Practices and the wider system to support the delivery of care to patients (including shared appointments between the Practices to aid delivery of enhanced access).
- Work with others in the PCNs and ICB to deliver positive change SW London NHS, including building capability for quality improvement across the Practices and within the PCNs, so developing system wide approaches to problem solving.
- Review and improve the Practices and PCNs’ digital maturity.

1. Understand demand, capacity and activity and drive improvements in:
2. Patient experience of access.

3. Operational efficiency including better matching capacity to need.

4. Staff experience at work.

5. Support population health management.

The post holder will work closely with Group and Site Managers in the Practices and the PCNs to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered
- Experience of working in general practice with a good working knowledge of general practice systems (including IT, digital and BI tools), processes, and procedures; or experience of successfully delivering change and improvement programmes in a patient (or customer) facing environment.
- Good communication skills and an ability to engage with a wide range of staff and stakeholders, building excellent and trusted relationships.
- Ability to extract, collate, visualise, and use data and insight to both drive change, and to understand and adapt programmes of work while in progress to allow learning to be integrated into delivery.
- Knowledge of Quality Improvement tools or a willingness to engage in the Quality Improvement training offers available to support the Transformation Team.
- Ability to understand, interpret and present complex and/or sensitive information.
- Ability to interpret qualitative and quantitative information from a range of sources.
- An ongoing commitment to championing diversity and inclusion and promoting actions to improve the experience of diverse groups, particularly promoting digital inclusion.
- An ongoing commitment to improving NHS services for the benefit of patients.

**Responsibilities**:
**Quality and Service Improvements and Responsibilities**:
The role will require experience in quality / service improvement with knowledge in transformational change and a good understanding of general practice systems and processes, including familiarity with general practice digital systems such as EMIS, Docman and how they can be used to deliver improvement.

This expertise will be used to:

- Work alongside practices to understand current challenges to effective service delivery and identify solutions, including existing best practice within the Practices.
- Support the planning and prioritisation of changes that will improve delivery of care and improve patient and staff experience in Groves Health
- Build relationships with the PCN to develop consensus for improvements and to ensure all staff (clinical and non-clinical) are fully engaged throughout the process of selection, implementation, deployment and monitoring/evaluation of change, facilitating peer challenge and learning.
- Support with the overall transformation plan for Groves Health, including identifying interdependencies, managing risks, considering the potential impacts on the wider organization, and determining resource requirements.
- Provide hands-on capacity and support to the group/site managers in implementing and sustaining agreed changes, including upskilling of practice team.
- Support staff to identify and participate in relevant training and/or professional development opportunities that will support achievement of agreed improvements, and optimisation of existing and new technology.
