Recall Coordinator

To be responsible for the recall of patients for annual reviews of long term/ chronic conditions (QOF) using the Practices current procedures. To be responsible for the administration of other screening recall systems (Bowel, Breast and cytology) To be responsible for the recall of patients for monitoring of other conditions and medications (not included in QOF), including but not limited to: Recall of patients who are prescribed High Risk medications. Recall of patients due for regular investigations such as DEXA Scans, PSA blood tests, X-Rays etc. Recall of patients prescribed medications administered on site e.g.

Denosumab and Prostap injections. To be responsible for checking the Accurx text system, on a daily basis and appropriately dealing with texts from patients. To run and work through regular audits as required, (including a monthly audit of Patients who live in a Care Home and are newly prescribed a Controlled Medication). To assist with the creation of searches and reports, as required, to support staff and clinicians with audit work.

To undertake any other duties assigned by the Admin Manager and Team Lead. To be responsible for the maintenance of Data Quality. To input patient data received from external sources into the EMIS Web clinical system, including; summarised medical records, hospital letters and hospital results etc. To cover for sickness/ absence within the Data Input and Docman Optimisation Team, when required, to aid business continuity.
