The post holder will be actively involved in the development, implementation, provision, monitoring and evaluation of the bladder and bowel service in conjunction with the Community Matron, Continence Advisors and administration team.

You will be expected to work autonomously to provide evidence based, comprehensive, advanced assessment / reassessments and treatments for patients in their own homes, residential care settings, day centres and nursing homes, in addition to clinic base settings.

You will be responsible for developing and updating evidence based care pathways. The post holder will be responsible for teaching carers and/or families how to manage treatment, management or containment programmes along with identifying staff training needs and support with training.

KEY RESPONSIBILITIES
- Provide skilled continence care to patients in a variety of community settings: patients’ own home, health centres, care homes, all community settings.
- Possess a good working knowledge of catheter care.
- Complete comprehensive bladder and bowel assessments, including assessment, planning, implementation and evaluation of patient care.
- Monitor, record, act upon and report any changes in patients’ conditions.
- To complete DATIX and ensure follow up for any patient identified with a pressure sore not currently being seen by the DN service, making appropriate referrals to TVN/ DN Team
- To effect change in practice to the care of patients with continence needs, using research. Evidence and Best Practice.
- To support multi-disciplinary teams, and allied services facilitating provision of an effective bladder and bowel service.
- To provide bladder and bowel reassessments to a range of patients with identified needs.
- Ensure documentation is accurate and up to date and written in line with the ICO policy and the Nursing and Midwifery Council’s recommendation.
- Demonstrate an inquiring approach towards continence care.
- Work with patients, carers and families to enable them to develop self-care skills with respect to continence management.
- To act as a resource for ICO colleagues, which may include preparing for and participating in training programmes.
- Ensure patient’s views are taken into account in the decision making process regarding their care.
- Educate patients, relatives and carers where appropriate (this may involve group training), encouraging self care.
- Initiate and follow up referrals where appropriate to relevant clinicians and or social services.
