This post is for a Senior Support Worker, who has a keen interest in Hepatitis C. This post will cover Lancashire and South Cumbria, with a working base in East Lancashire Hospitals. This post will cover a specific patient group with hepatitis C in the community, in hospital and prison.

This is a very exciting opportunity to join a small team in the delivery of care to Hepatitis C patients. The role is very varied day to day and covers a wide geographical area. It is a very rewarding role.

We care for patients in a variety of settings including on a clinical van, drug services, hostels, homeless shelters, food kitchens, prisons, probation services in all the localities as required.

Duties will include working with the Prison and roaming outreach team, to provide a coordinated and comprehensive service for clients receiving specialist services from the Lancashire and South Cumbria ODN Hepatitis C.

The role entails Hepatitis C testing, related diagnostic testing, delivery of results and coordination of services to all establishments and communities where the Lancashire and south Cumbria ODN has a responsibility

To provide full administrative support to the prison outreach and roaming team for Lancashire and South Cumbria including weekly Multidisciplinary Team Meeting and collecting and collating patient data.

To proactively monitor and track of the Hepatitis C patients ensuring that their journeys are processed in a timely and efficient manner in line with agreed pathways.

It is imperative that the post holder has a full understanding of the Hepatitis C Elimination process and can educate and advise key personnel on all aspects of Hepatitis C.

We are a small, friendly and enthusiastic team consisting of a Clinical lead, Programme Manager, Project support, Clinical nurse specialists, a senior support worker and administrator.

The main responsibilities of this role will be to support the Clinical Nurse Specialists in the diagnosis, care of patients with Hepatitis C. This will include patient finding, difficult venous blood taking, liver scans, and to attend network MDT meetings. This will also include supporting the Registered nurse in delivery of medication, monitoring and the establishment of sustained viral response post treatment.

You will support regular clinical van clinics, testing events and provide support as required across the ODN Network.
